Well Being Assessment questions to think about:

For Early Development ages 0 to 4:

  1. Is the child currently enrolled in an early childhood program such as a preschool or daycare?

  2. Have ave there been any reported behavioral problems, either at home or in preschool or daycare settings?

  3. Has the child been evaluated for or received any developmental interventions?

For Education ages 5 to 17:

  1. If the child/youth has changed schools in the past 6 months (other than advancing the next grade level), what is the reason why?

  2. Have there been any reported attendance problems?

  3. Have there been any reported behavioral problems at school?

  4. How has the child/youth performed academically?

  5. Has the child/youth been evaluated or received any educational interventions?

  6. Has the child/youth participated in any academic support programs and/or services outside of their regular school day?

  7. Has the child/youth participated in any recreational or social programs outside of their regular school day (e.g., sports, art, music, religious group)?
