Well Being Assessment questions to think about:
For Health for ages 0 to 4:
Is the child current with well-visits?
Has the child/youth received dental care and any necessary follow-up care?
If the child has any chronic physical health conditions, are they currently receiving the necessary care?
For Health for ages 5 to 17:
Is the child/youth current with medical well-visits or regular physicals?
Has the child/youth received dental care and any necessary follow-up care?
If the child/youth has chronic physical health conditions, are they currently receiving the necessary care?
If the child/youth has mental health needs, are they currently receiving the necessary care?
Has there been any reported behavioral problems (other than in school)?
Does the child/youth feel that they have at least one positive adult role model other than you, their CASA Volunteer Advocate?
Does the child/youth indicate that they have skills or excel at certain activities (e.g, art, school, sports)?
